The Advanced Educational Program in Oral and Maxillofacial surgery at Cook County Hospital is a combination of 4 and 6-year program. The Cook County OMFS Residency Program participates in both the PASS/MATCH. The program accepts two residents each year, one in 4-year certificate track and another in 6-year medical school track. This program provides training in all aspects of Oral & Maxillofacial Surgery. Cook County Hospital is a busy level I trauma center in inner-city Chicago. The OMFS clinic sees over 16,000 patients on an annual basis. Residents are eligible for board certification upon completion of the program. The program provides exposure to broad scope Oral & Maxillofacial Surgery. A strong dentoalveolar and implant experience is also provided.

Applicants
Cook County OMFS participates in PASS/MATCH and has two resident and four internship positions available each year.
Follow the steps below to be considered for a residency position at Cook County Oral and Maxillofacial Surgery:
1) The Cook County OMFS program participates in the PASS/match and does not require any additional application forms.
2) Submit the following to ADEA PASS
a) Official Undergraduate and Dental School Transcript
b) Official NBDE report (Part I and Part II if applicable) or INBDE report (if already completed)
c) Three letters of recommendation
d)A copy of the (NBME) CBSE exam score report
e) 2x2 inch photo of the applicant
5) Meet the following requirements:
a) Must have passed Part I of the National Dental Board Examination or the Intigrated National Dental board Examination
b) Must be a graduate of a U.S. or Canadian dental school, or a dental school with equivalent training.
c) Must be proficient in the English language.
d) Must report postdoctoral Dental Match Program Code in PASS application.
Prior to enrollment in the program you:
a) Must have passed Parts I and II of National Dental Board Examination or the Intigrated National Dental Board Examination
b) Must have earned a D.M.D/D.D.S. degree, or equivalent
c) Must be eligible for state dental license. Temporary or permanent Illinois license is required
d) Must have taken the (NBME) CBSE examination
