Transcript Evaluation and Instructions

o   Notarized Color Copy of Official Dental School Transcript

o   Certified English translation report by certified translation service (This has to be done only if the Official Dental Transcript is in another language).

 

Letters of Evaluation Instructions

To be uploaded on ADEA PASS Requested from ECE Educational Credential Evaluators

o   Official three (3) current letters of professional reference from a dental school dean, dental faculty, dental department chairs, dental directors, supervise faculty, or dentists who have supervised your work.

TOEFL Requirement Instructions

To be emailed directly to Resident Coordinator Jdelisca@csmiles.org

School Code: 2285 (TOEFL scores expire after 2 years and must be from February 8, 2023, or later)

  • Official TOEFL score report (Minimum passing score: 85 or higher, under the old system that will be in place until 2027, or the new system a score of at least 4.0 or but may be as high as 4.5).
  • It is mandatory for applicants whose dental degree was completed at school where English is not the official lenguage

Must have a United States Social Security number to apply.

Will also accept Certified of Naturalization or F1 Visa holder or Employment Authorization Card with the United States of America Social Security Number to apply.

The ADAT exam is NOT required.
