Recommendation Letters/Personal Evaluation Forms (PEFS): We require 3 clinical recommendation letters.
2 of the clinical letters must be from
- Faculty that can attest to your general dentistry clinical skills.
- Examples: Operative/restorative/Comprehensive Care Clinical Faculty/Group Practice Leaders (This includes letters from general dentists, pediatric dentists, and prosthodontists)

This program will consider applicants who graduated, or plan to graduate, from a non-CODA accredited dental school: No
