Letters of Evaluation Instructions
Candidates are required to submit an institutional evaluation form in addition to four personal evaluation forms. The personal evaluations may come from a source of the applicants choosing.

- Canadian Citizen
- Canadian Permanent Resident
- Non-US Citizen/Resident (program offers sponsorship)
Additional Information
- Official NBDE results must be submitted to PASS directly for application review. If your examinations took place prior to 2012, official NBDE results must be forwarded directly to our institution at sodadmit@ohsu.edu
- ADAT scores not required, but preferred
- Preference is given to candidates whose dental school graduation occurred 4+ years prior to the PASS application deadline
- Historically, students could opt to pursue an optional masters of science degree in addition to the endodontic certificate. This option is currently unavailable.
