    • Unofficial transcripts are sufficient at time of application. Original official transcripts in an envelope sealed by the institution are required after acceptance into the program.

Three Professional Evaluation Forms (PEF): one from the Oral Surgery Chair, one from the Oral Surgery Program Director, and one from an Oral Surgery Faculty Member

Applicants are eligible to enroll if they are:

  • US Citizen
  • US Permanent Resident
  • To be eligible for admission, applicants must be a United States citizen or a permanent resident. Additionally, applicants must graduate from an American Dental Association (ADA) or Commission on Dental Accreditation of Canada (CDAC) accredited Dental School (minimum 4 year program).
