- Unofficial transcripts are sufficient at time of application. Original official transcripts in an envelope sealed by the institution are required after acceptance into the program.
Letters of Evaluation Instructions
Three Professional Evaluation Forms (PEF): one from the Oral Surgery Chair, one from the Oral Surgery Program Director, and one from an Oral Surgery Faculty Member

Applicants are eligible to enroll if they are:
- US Citizen
- US Permanent Resident
- To be eligible for admission, applicants must be a United States citizen or a permanent resident. Additionally, applicants must graduate from an American Dental Association (ADA) or Commission on Dental Accreditation of Canada (CDAC) accredited Dental School (minimum 4 year program).
