All applicants must submit a VA application for residents (VA form 10-2850b) which can be obtained on line or by emailing: james.melillo@va.gov.  The application must be mailed to: Dr. James V. Melillo, Dental Servive 160, VA New York Harbor Healthcare System, Manhattan Campus, 423East 23rd St. ,New York, NY 10010.  All other application information is submitted via PASS.  THE PASS APPLICATION AND THE VA APPLICATION MUST BE RECEIVED BY THE PROGRAM BY OCTOBER 1.

 

Note: APPLICANT MUST BE A U.S. CITIZEN AND GRADUATE OF AN ADA-APPROVED U.S. OR CANADIAN DENTAL SCHOOL.  ALL MALE APPLICANTS MUST BE REGISTERED WITH THE U.S. SELECTIVE SERVICE SYSTEM.
